It actually uses a simple reinforcement. You are not using it correctly. Your dog is not associating the barking with the noise, and no, it does not need to go off every time. It is actually a bit harder for the dog to learn that way but is much more effective once they learn. I got one and after about a week my dog was wonderfully trained to not bark at the door.
